Information about Advanced Care Planning
If you were unwell, and unable to speak for yourself, do the people close to you know your wishes for health care and personal decisions? Advance care planning is the process of thinking about, talking about, and documenting your wishes for health care if you are unable to speak for yourself. All Albertans over the age of 18 should have a personal directive.
Advance care planning is important at all times. The COVID-19 pandemic has brought the importance of knowing peoples wishes and values regarding their health care to the forefront.
This session will provide:
• Information on advance Care planning and the importance & role of Personal Directives
• An explanation of Goals of Care and what they mean for you
• Information on resources and where to find them will be included along with time for questions
